Key Responsibilities

• Conduct home health visits including Start of Care and Follow-Up visits for an expected caseload of 5-10 patients per day, equating to approximately 30 productivity points per week
• Travel throughout the assigned territory (Benton and Franklin County), with expected weekly drive time of approximately 100-150 miles; mileage is reimbursed at the IRS standard rate from patient home to patient home
• Document all patient care within 24 hours of each appointment using the HomeCare HomeBase (HCHB) EMR system
• Attend periodic office/clinic meetings as required
• Accept all patients on assigned schedule regardless of geographic distance from previous appointment; all overtime must be pre-approved

• Active, unencumbered Physical Therapist license in hand (no pending licenses accepted)
• Minimum 1 full year of recent adult Home Health experience; first-time home health candidates will not be considered
• Proficiency in HomeCare HomeBase (HCHB) EMR used within the last 6 months, or willingness to complete a retraining course during compliance
• CPR/BLS certification required
• Reliable personal vehicle, valid driver's license, and agency-required auto liability insurance
• Willingness to travel up to 75 miles from the assigned care center
• Open to on-call rotation as discussed with the hiring manager during the interview
• Locals are welcome and accepted at the same rate as travelers

• Experience managing a high-volume home health caseload with strong time management skills
• Familiarity with OASIS documentation (required for RN roles; beneficial for therapy staff)
• Comfort with extended drives between patients in a multi-county territory

• Join a growing home health program with an increasing patient census, offering a dynamic and autonomous clinical environment
• Mileage reimbursed at the IRS standard rate, and if patient volume is lower than expected in a given week, there may be an opportunity to float to a nearby clinic to maintain hours
